First time patients: Please fill out the applicable intake form before you arrive for your treatment. Bring lab work, diagnostic reports, and any other relevant information or have your practitioner(s) email the information to us in advance.
Insurance: We operate independently of all insurance networks, allowing us to prioritize delivering the highest quality care to our patients. If you would like a super bill please ask before your treatment.
Payment: We require full payment upfront at our office after each treatment. We accept cash, check, venmo, or paypal.
Cancellation Policy: Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the therapists’ day that could have been filled by another patient. As such, we require 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice will be charged $75.
